Job Description

We are currently seeking a 6-month Temporary OAS Intake Coordinator and Geriatric Care Manager who will be responsible for serving as the initial contact person for Older Adult Services (OAS) client inquiries as well as maintaining a caseload of geriatric care management clients. The OAS Intake Coordinator and Geriatric Care Manager will provide the following services: telephone screening of all potential Older Adult Service clients; information, assistance, and referral; comprehensive assessment; care management and telephone consultation. We are looking for someone whose situation fits the 6 month contract nature of the role and who is excited by the idea of working with older adults.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Intake Responsibilities (Approximately 50% of the position)
  • Respond to intake calls for the OAS department. Perform a telephone screening to determine if potential clients are appropriate for an internal department or an outside provider. In some cases, further in-person assessment may be required.
  • Work closely with the OAS Co-Directors to facilitate assignment of clients to OAS staff and to ensure the successful transition from intake to ongoing services.
  • Maintain information on community resources. Refer people to outside services as appropriate.
  • Provide short-term telephone-based consultation to older adults and their families, occasionally including telephone-based advocacy.
  • Admit new clients to Salesforce electronic medical record database.
  • Keep current on all JFS programs and resources.
  • Care Management Responsibilities (Approximately 50% of the position)
  • Provide comprehensive assessment of older adults’ physical and mental health, home safety, social needs, cultural needs, and financial status.
  • Develop care plans in collaboration with clients and help clients achieve their care plan goals.
  • Serve as a client advocate and contact clients’ healthcare providers, community agencies, etc., to ensure clients’ needs are met.
  • Coordinate services with Older Adult Services and JFS staff as well as multiple service providers
  • Provide information, assistance, and referral to senior clients and their families.
  • Provide ongoing oversight of each assigned case, maintaining regular contact with client (including home visits, telephone checks, annual reassessments, etc.).
  • Maintain accurate and complete documentation by recording all client contact in the Salesforce database, including demographic information and ongoing client and collateral contact.
  • Comply with all contractual requirements regarding service provision and documentation if working under contract auspices (e.g. City of Seattle, Claims Conference).
  • Attend and participate in monthly OAS clinical consultation and team meetings and other meetings as requested.
  • Develop and maintain clinical skills.
  • Perform other duties as assigned.

 
QUALIFICATIONS:

  • A master’s degree in social work or related field and one year of paid social service experience is preferred; a bachelor’s degree in social work or related field and two years of paid social service experience will be considered.
  • Two years’ experience in older-adult programming is preferred.
  • Licensure as a social worker or mental health counselor is preferred.:
  • Must have a valid Washington State Driver’s License and access to an insured vehicle.
  • Excellent written and verbal communication skills.
  • Excellent organizational skills with the ability to balance and prioritize multiple tasks and requests and meet deadlines in a professional manner.
  • Excellent attendance record.
  • Strong knowledge and understanding of care management, trauma-informed care, and client record keeping systems.
  • Ability to multitask in a fast-paced environment with excellent communication skills.
  • Ability to display sensitivity, tact, and responsiveness in various situations and maintain a high level of confidentiality.
  • Must be able to work effectively and compassionately with clients who are frail or have physical or cognitive disabilities.
  • Must have knowledge of community resources; knowledge of Jewish community resources preferred.
  • Demonstrated excellent interpersonal skills.
  • Ability to evaluate problems and effectively solve them in a timely manner. Ability to respond to concerns with empathy and inclusivity.
  • Strong interview and assessment skills.
  • Knowledge of Jewish culture and religion preferred.
  • Proficiency in Microsoft Office products, particularly Word, Outlook, Excel, and Teams.
  • Ability to use an electronic database and enter data with a high level of accuracy is also required.

 
SALARY AND BENEFITS:

  • The pay for this position is $29.38 – $35.91.
  • This position is eligible for medical insurance, the Employee Assistance Plan, and Seattle Sick Time.
